Start with what you need
Which dental appointment is right for you?
You do not need to diagnose the problem yourself. Tell us what you would like help with, and we can guide you towards an appropriate appointment.
Check up and clean
A routine dental examination, preventive care or a fresh start after time away. Tell us about any new concerns when booking so we can plan your visit.
Pain or a broken tooth
Call us about toothache, swelling, an injury or a lost filling. Explain your symptoms and when they started so the team can discuss urgency and availability.
Invisalign consultation
Interested in straighter teeth? Arrange a consultation to discuss your goals and the assessment process. Treatment suitability depends on an individual clinical assessment.
Cosmetic or implant advice
Tell us whether you want to discuss the appearance of your smile or options for missing teeth. We can help you choose the appropriate consultation.
Need help urgently?
Phone (03) 9533 8488 for a prompt discussion about dental pain, swelling or injury. Online messages may not be read immediately, and appointment availability varies.
If you have difficulty breathing or swallowing, rapidly worsening swelling, or uncontrolled bleeding, seek emergency medical care. Call 000 for a life threatening emergency. See Healthdirect advice about toothache and swelling.

Email an enquiry or request a dental appointment
Use the form below to ask a question or request an appointment by email. Tell us what you would like help with and which days and times suit you. For a general question, select “No – enquiry only”.
Submitting this form does not reserve or confirm an appointment. Our team will contact you to discuss your request and available options. Please check that your email address and telephone number are correct.
For urgent pain, swelling or an injury, or to change an approaching appointment, please call (03) 9533 8488. Messages may not be read immediately. Keep your initial message brief; the medical history PDF is separate from this enquiry form.
A conversation before a decision
What happens at your first visit?
We take time to get to know you, listen to your concerns and understand what matters to you. There is no need to feel embarrassed if it has been a while since your last dental appointment.
- We review your medical and dental history, including medicines, allergies and relevant health changes.
- Your dentist assesses the concerns relevant to your visit and explains any recommended investigations. Digital X rays are used when clinically indicated.
- We talk through the findings, your options, likely fees and next steps, so you can make an informed choice.
What can be completed on the day depends on the appointment booked, your clinical needs and the time available. Ask our team what your visit includes when arranging it.

Before you come in
Prepare for your dental appointment in 3 simple steps
1. Complete your medical history
Download the current three page fillable Medical History Form. Complete it on a desktop or laptop using a compatible PDF app, save your answers and bring the saved copy or a printout. You can also print a blank form and write by hand.
Prefer to complete it at the practice? Please arrive at least 10 minutes early, allowing extra time if you would like assistance. Saving the PDF does not send your answers to us. Call for the correct practice email address if you wish to email your completed copy.

2. Have the useful details ready
Bring a current medication list, allergy information and your private health insurance card if applicable. Tell us about relevant previous dental records so we can discuss obtaining them with your permission. Note down your questions, symptoms and priorities.
3. Check your confirmation and plan your journey
Check your appointment date, time and visit type before travelling. Our Contact page has the map, parking and transport information. Phone ahead about access or assistance requirements. Follow any individual preparation instructions from your dentist; do not stop prescribed medicines or fast unless specifically advised by your treating clinician.

Tell us what helps
A more comfortable visit starts with a conversation
If you feel nervous, have a sensitive mouth or have had a difficult experience before, please tell us when booking. We will listen and discuss how to make your appointment more manageable.
You can ask about breaks, a slower pace and comfort options suited to your visit. Let us know if you need extra assistance or want to discuss bringing a support person. We want you to feel able to ask questions at every stage.
For a routine question before booking, use the enquiry form on this page. Sending an enquiry does not reserve or confirm an appointment.

Helpful answers before you book
Dental appointment questions
Which dental appointment should I choose?
For a routine examination or preventive care, ask for a check up and clean. If you have pain, swelling, a broken tooth or a new concern, tell our team before choosing a routine booking. For Invisalign, cosmetic dentistry or implants, explain your goals so we can arrange the appropriate consultation. If the online options do not match your needs, phone us rather than choosing an unrelated appointment.
How do I book a dental appointment online?
Select Book online to open our booking service in a new tab. Choose the reason for your visit, review the available times and follow the booking steps. Check the patient details, appointment type, date and time before finishing. Keep the confirmation provided by the service. If you are unsure whether your booking went through, call the practice to check.
What if I cannot find a suitable time online?
Call (03) 9533 8488 during office hours. The team can discuss your needs and availability. Online availability can change and does not show every possible arrangement. We cannot promise a particular time or a same day appointment, but we will help you understand the options. For a routine request you can also use the enquiry form on this appointment page.
What happens at my first dental appointment?
We begin by listening to your concerns and reviewing your medical and dental history. The dentist will assess the areas relevant to your visit and explain any recommended investigations. Digital X rays are taken when clinically indicated, rather than automatically for every patient. You can ask about findings, treatment alternatives, likely costs and the next steps before deciding. Cleaning or treatment on the day depends on your booking, clinical needs and available time.
How early should I arrive, and what should I bring?
If you need to complete paperwork at the practice, arrive at least 10 minutes early and allow extra time if you would like assistance. Bring your completed medical history form if you have prepared it, a current medication list, details of allergies and relevant health changes, and your private health insurance card if applicable. Let us know about relevant previous dental records so we can discuss how to obtain them with your permission.
Can I fill in the medical history form before my visit?
Yes. Open the current fillable PDF using the link above. For the easiest experience, use a desktop or laptop and a PDF app that supports fillable forms. Save your answers and bring the saved file or a printed copy. You can also print a blank form and complete it by hand. Saving the PDF does not send it to the practice. Call us for the correct email address if you wish to return it by email.
I feel nervous. Can you help me prepare?
Please tell us when booking, even if you have not visited a dentist for a long time. Explain what worries you and what has helped before. We can discuss a gentler pace, breaks and comfort options appropriate for your appointment. You can ask questions before agreeing to treatment. Let us know beforehand if you need assistance or would like someone to accompany you.
Can I ask about appointment costs before booking?
Yes. Tell the team which visit you need and ask what is included in the appointment fee. The cost of further investigations or treatment depends on your individual needs and should be discussed before proceeding. Private health insurance benefits depend on your policy and available limits. Ask about an estimate, item numbers and payment options rather than assuming that a consultation or all treatment is covered.
How do I change or cancel a booking?
Please phone the practice on (03) 9533 8488 as soon as you know your plans have changed. Have the patient name and appointment date and time ready. Our team can discuss the arrangements and any applicable booking conditions. A message asking for a different time does not confirm a replacement appointment; check the revised details with us.
Do I need to prepare differently for a specific treatment?
Follow any personal instructions given by your dentist or our team. Do not stop prescribed medicines or fast unless your treating clinician has specifically advised you to do so. If you are unsure about eating, medicines, transport or recovery arrangements, call before the appointment. Routine examination preparation can differ from instructions for sedation or a procedure.
What are your appointment hours?
Office hours are Monday 8.00 am to 7.00 pm, Tuesday to Thursday 7.30 am to 4.00 pm, and Friday 7.30 am to 12.30 pm. Clinical hours and appointment availability may vary. Public holidays can affect opening. Always use your confirmed booking time and contact the practice if you need clarification.
Where can I find parking, directions and access information?
Our practice is at Level 2, Suite 219, 566 St Kilda Road, Melbourne VIC 3004. Visit our Contact page for the map, designated free patient parking on Queens Lane and public transport information. Call before visiting if you have access or assistance requirements so we can discuss the building arrangements with you.
